Responsibilities:

As a Community Health Advisor, you will act in a liaison role with retiree health plan members to conduct a home-based questionnaire and provide members with information regarding community-based organizations and health plan benefits that can support their needs. The home visit takes about 30 minutes and uses a standard questionnaire from the Centers of Medicare and Medicaid.
