General Summary: 
Openings for Field-Based Community Health Advisors in Southeast Michigan. You must be able to work between 20-40 hours per week. As a Community Health Advisor, you will act in a liaison role with retiree health plan members to conduct a home-based questionnaire. This position has a base pay in addition to bonus opportunity based on completed visits/questionnaire. Base pay is $13.79 /hour. In person paid training at $10.00/hr. If chosen be prepared to complete an online application and meet employer for a face to face interview. 

Responsibility:
•    Complete in-home visits to identify needs and provide resources
•    Call and schedule visits with members to participate in the home-based questionnaire
•    Provide a list of helpful resources to the member
•    Complete electronic documentation (calls, scheduling, questionnaire, etc.

Qualifications:
•    High School Diploma / GED (or higher)
•    Must be able to work independently
•    Valid driver’s license and automobile insurance
•    Access to reliable transportation that will enable you to travel to members’ homes within a designated area of Southeast Michigan
•    Community Health or Home Health experience a plus
•    Bilingual: English and Spanish a plus 
•    Ability to use technology provided in the field
