You must wear business attire (no jeans/gym shoes). This is a first level interview. Capacity is limited. 
Durations of recruitment events vary, so please be prepared to spend up to the full day with us. 

General Summary:
As a Community Health Advisor, you will act in a liaison role with retiree health plan members to conduct a home-based questionnaire and provide members with information regarding community-based organizations and health plan benefits that can support their needs. This position has a base pay in addition to bonus opportunity based on completed visits/questionnaire. The home visit takes about 30 minutes and uses a standard questionnaire from the Centers of Medicare and Medicaid.

Representative Responsibilities:
•    Complete in-home visits to identify needs and provide resources
•    Call and schedule visits with members to participate in the home-based questionnaire
•    Provide a list of helpful resources to the member
•    Complete electronic documentation (calls, scheduling, questionnaire, etc.)
Qualifications:
•    High School Diploma / GED (or higher)
•    Knowledge of culture and values in the local community
•    Must be able to work independently
•    Valid driver’s license and automobile insurance
•    Access to reliable transportation that will enable you to travel to members’ homes within a designated area of Southeast Michigan

   •   Your valid (non-expired) Michigan ID or Driver’s License 
   •   Your Social Security Card or Passport 
   •   Several copies of your resume 
   OPTIONAL: 
   •   A copy of your DD214 or letter from the VA if you are a veteran 
   •   Proof of public assistance income like an award letter from DHS or Social Security 
   •   Proof of any income earned from a job in the past 6 months (check stub) 
   •   A copy of your layoff notice or Unemployment documentation, if you are laid off
